1. SCOPE OF SERVICES

Hankins Hormonal Health (“Practice”) provides a range of services focused on Bio-Restorative Medicine, Hormonal Optimization, Functional Wellness, and Performance Enhancement Protocols. These services may include, but are not limited to, clinical consultations, laboratory analysis, and the prescription of specialized therapeutic agents.

  • Urgent Notice: Hankins Hormonal Health is not an emergency medical provider. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. We do not provide acute crisis intervention or urgent care services.

  • No Doctor-Patient Relationship: Use of this Site or booking an "Initial Consultation" does not establish a doctor-patient relationship. Such a relationship is only formed after a clinical evaluation, signed treatment agreement, and the provider’s formal acceptance of your case.

  • Service Evolution: Practice reserves the right to modify, add, or remove specific treatment modalities at any time without prior notice. All new services added to the Site are automatically governed by these Terms.

  • Educational Resources & Newsletter: The Practice provides a newsletter, blog articles, and clinical briefings for educational purposes. Subscription to these communications or interaction with blog content does not constitute medical advice or a doctor-patient relationship. These resources are intended to support, not replace, the relationship between you and your primary healthcare provider.

 

2. MEDICAL DISCLOSURES & "OFF-LABEL" TREATMENTS

  • Provider Discretion: All treatments, including hormonal therapies, peptides, and nutritional protocols, are prescribed at the sole discretion of the Provider based on clinical necessity.

  • Off-Label Use: You acknowledge that many therapies provided by the Practice are prescribed for "off-label" or "investigational" purposes to optimize health and performance. These uses may not be the primary indication approved by the FDA for the specific agent used.

  • Compounded Medications: The Practice often utilizes highly specialized formulations from accredited compounding pharmacies (503A/503B). You acknowledge that these custom formulations are not "FDA-approved" in the same manner as mass-produced commercial drugs.

 

3. ATHLETE RESPONSIBILITY & ANTI-DOPING COMPLIANCE

  • Strict Liability: If you are an athlete subject to the World Anti-Doping Agency (WADA), USADA, or any other governing body, you acknowledge that many optimization therapies (including Testosterone and various Peptides) are Prohibited Substances. * No Practice Liability: It is your sole responsibility to verify the status of any treatment with your governing body before beginning therapy. Practice is not liable for athletic disqualification, loss of eligibility, or reputational damage resulting from prescribed treatments.

 

4. CONTROLLED SUBSTANCES & REGULATORY COMPLIANCE

For any therapy involving federally regulated substances (including, but not limited to, Testosterone):

  • No Guarantee of Prescription: A paid consultation is for an evaluation only. No medication will be prescribed without objective laboratory evidence and clinical justification.

  • Laboratory Monitoring: Ongoing therapy is strictly contingent upon regular blood chemistry monitoring.

  • Regulatory Reporting: Practice complies with all 2026 DEA and State Prescription Drug Monitoring Program (PDMP) requirements.

  • Diversion: You agree not to share, sell, or distribute any medication. "Doctor shopping" or seeking overlapping prescriptions from other providers is grounds for immediate termination.

  • Compliance: Practice reports all regulated prescriptions to the State Prescription Drug Monitoring Program (PDMP) as required by 2026 federal law.

 

5. FINANCIAL & SUBSCRIPTION POLICY

  • Concierge Model: Practice operates as a direct-pay, concierge entity. We do not participate in any insurance programs. You agree not to submit our invoices for insurance reimbursement.

  • Consultation Fees: The $50 deposit is non-refundable. The $250/100 consultation fees cover the medical evaluation only and do not include the cost of medication or labs.

  • Recurring Programs: Monthly fees cover clinical oversight and the management of your optimization plan.

  • Cancellation: Written notice is required 30 days prior to the next billing cycle. No refunds are granted for services or medications already rendered.

 

6. LIMITATION OF LIABILITY & ARBITRATION

  • Unsecured Communications: While we utilize HIPAA-compliant portals for clinical data, any communication initiated by the patient via standard email or SMS/Text is not 100% secure. By choosing to communicate via these channels, you acknowledge the inherent privacy risks and authorize the Practice to respond via the same medium. The Practice is not liable for data breaches occurring through unsecured third-party communication channels.

  • Clinical Outcomes: Practice is not liable for outcomes resulting from the Patient's failure to disclose full medical history, provide accurate laboratory data, or follow prescribed protocols.

  • Binding Arbitration: Any legal disputes, claims, or controversies arising out of or relating to these Terms or the use of this Site will be settled via private, binding arbitration in the State of Ohio.

  • Third-Party Content & Links: Our blog and newsletter may reference clinical studies or third-party health resources. The Practice is not responsible for the accuracy, clinical validity, or privacy practices of external websites or researchers. Reliance on any information provided by third parties linked through our Site is solely at your own risk.
